NIHSS Score and NIH Stroke Scale: What the Results Mean

The NIH Stroke Scale (NIHSS) is a standardized neurological examination used by trained healthcare professionals to describe stroke-related deficits. It does not diagnose stroke by itself, replace brain imaging or predict one person's recovery.

Emergency: Call 911 immediately for sudden facial drooping, arm or leg weakness, speech difficulty, vision change, severe imbalance, a sudden severe headache or another new neurological symptom. Do not wait for an online score or for symptoms to improve.

What does NIHSS mean?

NIHSS stands for National Institutes of Health Stroke Scale. Clinicians use the same structured examination to record findings, communicate stroke severity and monitor changes over time. The score is most useful when it is performed and interpreted by trained professionals together with the full examination and imaging.

What does the NIH Stroke Scale assess?

The examination evaluates several areas of neurological function:

  • level of consciousness and ability to answer questions or follow commands;
  • gaze and visual fields;
  • facial movement;
  • arm and leg strength;
  • coordination;
  • sensation;
  • language and speech; and
  • attention or neglect.

NIHSS score range: 0 to 42

The total NIHSS score ranges from 0 to 42. In general, a higher total reflects more observed neurological deficits during that examination. Hospitals and studies may group scores differently, so the treating team should interpret the total together with the individual item scores, symptoms, imaging and time since onset.

A score of 0 does not prove that no stroke occurred. It means the scale did not record a measurable deficit at that examination.

What does a low NIHSS score mean?

A low score does not rule out stroke and does not automatically mean the symptoms are nondisabling. Posterior-circulation symptoms, severe imbalance, vision loss, isolated language problems or loss of important hand function may matter greatly even when the total is low.

People with sudden symptoms still need urgent medical assessment. The NIHSS should not be used as a home test or as a reason to delay emergency care.

How clinicians use NIHSS

Healthcare teams may use NIHSS to establish a baseline, describe changes, support communication and contribute to treatment or research decisions. It is only one part of stroke assessment. Brain imaging, symptom timing, medical history and the rest of the neurological examination remain essential.

Can an NIHSS score predict recovery or life expectancy?

No single NIHSS score can predict one person's recovery, disability or life expectancy. The score can help describe observed severity, but outcome also depends on stroke type and location, speed of treatment, age, other health conditions, complications and rehabilitation.

Frequently asked questions

Is NIHSS a home stroke test?

No. It is a clinical scale designed for standardized administration by trained professionals. For sudden possible stroke symptoms, call emergency services rather than attempting to calculate a score.

Is NIHSS the same as B.E. F.A.S.T.?

No. B.E. F.A.S.T. is a public warning-sign reminder. NIHSS is a detailed clinical examination used by healthcare professionals.

Can the NIHSS score change?

Yes. The score may improve or worsen as symptoms change, which is why clinicians may repeat the examination. A change should be interpreted by the medical team in context.
