The Action Research Arm Test (ARAT)

22 Jul 2024

The Action Research Arm Test (ARAT) is an index used to evaluate specific changes in upper limb function in individuals with paralysis due to sustained damage to the cerebral cortex (Lyle, 1981). It evaluates the client's ability by asking them to manipulate objects of different sizes, weights, and shapes, and is therefore considered a way to specifically test the limits of arm movement (Platz, Pinkowski, Kim, di Bella, & Johnson, 2005).
It has been verified that compared with other scales, the Upper Limb Action Research Scale (ARAT) has good convergent validity, discriminant validity and concurrent validity in stroke patients.
The 19 assessments in the ARAT are divided into 4 subtests (Grab and Place GRASP, Grasp and Place GRIP, Pinch and Place, GROSS MOVEMENT). The performance of each project is evaluated through the following 4 levels:
3 points: The test was completed perfectly.
2 points: Test completed. But it will obviously take more time and/or be more difficult than the unaffected side. The movements are slow and clumsy.
1 point: Completed part of the test. Ability to pick up or lift test items from work platform but unable to place them in correct location. For example, in "Pour water from one cup to another
In the test item "a cup", he can pick up a cup, but cannot pour water into another cup. In the fourth major assessment item (simple gross motor assessment), being able to lift his arm is scored as 1.
0 points: Unable to complete any part of the test task.
If item 1 of the subtest (the most difficult item) is scored as 3 points, the patient will receive full points for all items of that subtest. Then go to the next subtest and continue testing.
If the score on the first subtest is less than 3 points, the second subtest (the easiest of each subtest) is used.
·If the second item of the subtest scores 0 points, the total score of the subtest is 0 points, and the test should be continued to the next subtest.
